transfer into the state of Louisiana from any state in the United
States must fill out and return this application to the Louisiana
State Board office.

The following states MUST provide an affidavit because their state does not have
online license verification: |

- Kentucky
- Mississippi
- New Hampshire
- New York

- North Carolina
- Oklahoma
- Washington
- Wyoming

The following states DO NOT have to provide an affidavit; the state board has an
online license verification that we can use:
